Copyright declaration is an official document which has to be completed every time you order CD/DVD/vinyl data carrier in any format at GZ Media. The document explains who the owner of the mechanical rights to the ordered material is and if it is protected by any of the BIEM member organization (e.g. OSA, SACEM, GEMA, NCB, etc.).
For better clarity, please complete using our template.

Payment by card
If you decide to pay the pre-payment invoice by card, please send us a completed and signed Payment Card Payment Authorization form (to be found below).
The amount will be subsequently debited from your bank account and your order will be released for production.
